How do I update my name/date of birth (age)?

This article will show you how to submit a date of birth or name change request for optimal resolution time. If your name or date of birth is just a small typo you will not need to attach a piece of government ID. 

Instructions

1. Subject Field: Information Update – Name
2. List the following information in your email/request.
   • • Current Name on the account:
     • New name:
     • Incorrect Date of Birth:
     • Correct Date of Birth
3. You will need to attach a piece of government ID with the old name and one with the new name (Driver’s license, marriage certificate, etc).

I recently got married and my last name has now changed. Please see the necessary info below:

• Name on account: John Smith
